How the Savings Plan Works
Getting started is simple. Choose the membership at your next visit or call our office to enroll over the phone. Once your annual fee is paid, your benefits activate immediately and stay active for a full year. When you come in for covered services, there are no claims to file and no reimbursements to wait on — you simply enjoy your included exams and screenings and receive your member discount on additional treatment. When your year is up, renewing keeps your coverage going without interruption.
Dental Savings Plan Terms & Conditions
- The membership fee is non-refundable and due in full upon enrollment.
- The membership plan cannot be combined with any dental insurance, payment plan, discount, or credit.
- Any prior balance must be paid in full at the time of membership enrollment.
- In-house savings membership covers one year starting from the enrollment date.
- Plans do not apply to any treatments scheduled before enrollment.
- Unused appointments and or benefits will not be refunded or carried over to the next year.
- The in-house savings plan only covers services performed in-house.
- Payment for treatment is requested in full at the time of service.
- The membership renewal fee is subject to change annually.
- Members must notify Radiant Dentistry of Woburn 48 hours in advance if canceling a scheduled appointment. A fee (25% of total treatment) may be charged for missed appointments.
